What We Need From You

  • Information of all the medications you are taking, including non-prescription drugs, vitamins, and herbal products.

  • Information on how many and which different doctors are prescribing medication to you.

  • We ask that you DO NOT have your medication filled at more than one pharmacy. This practice is potentially dangerous!
